Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.
When you come for your appointment, please bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- Please provide the images on a CD, "Not just the report," Or a login to access the imaging remotely. Your doctor will need to look at the images to help determine the diagnosis, and treatment reports alone are not sufficient
- List of medications (if any)
